Stonerise Home Health and Hospice cares for patients across a diverse range of ages, conditions and needs. Our home health clinicians help assess patients and teach them how to manage the transition home after a hospital stay, or how to manage new or chronic diseases such as heart failure, COPD, diabetes, and cardiovascular or neurological illnesses.
Stonerise Hospice provides comfort and assistance to patients and families at home so they can make the most of their time together. We give patients and their loved ones the respect, dignity and comfort they need during these delicate times.
Home Health
Our clinician-led care keeps patients independent and at home.
Often, home health care is the last step in recovery after a hospital or skilled nursing facility stay. Our care can mean the difference between a patient maintaining independence at home or being admitted or readmitted to a hospital.
Regarding cost, home health care is inexpensive, considering the alternative of being admitted to a hospital. Medicare covers 100% of home health care, and most private insurers cover it as well.
Our team of highly skilled clinicians includes nearly 200 team members across 25 counties. Our nurses, physical therapists, occupational therapists, speech language pathologists, social workers and home health aides are committed to engaging and empowering patients toward maintaining independence at home.
We are able to provide technical services like wound care, injections and infusion therapy at home as needed and collaborate with physicians to keep patients safe, healthy and at home. We help patients manage chronic conditions between visits to their family doctor and specialists, providing the extra support and coordination that patients need for the best health outcomes.

Stonerise Hospice provides comfort and assistance to patients and families at home so they can make the most of their time together. We give patients and their loved ones the respect, dignity and comfort they need during these delicate times.
The hospice team supports our patients and their families with intentional, high-quality hospice care throughout southeast Ohio.
We do this through a number of clinician-led and non-clinician services:
Companionship for a loved one to ease any extra stress on family members
Spiritual and emotional support for family members
Skilled nursing and medication
Skilled therapy as needed
Counseling and bereavement support services
Home care (bathing, dressing, light house cleaning, meal preparation, laundry, etc.)

Our in-home clinicians created an individualized care plan for her with the goal of getting her back to her routine as quickly as possible, and worked closely with her physician to understand her health needs. After three months of therapy, she was back to her routine and back to health.
